Reflection: Unintentional Weight Loss Case

The case about unintentional weight loss was very helpful in learning about the causes behind unintentional weight loss, especially Alcohol Use Disorder (AUD) in addition to practicing writing my very first SOAP note. Prior to this assignment, I have never encountered SOAP notes before, let alone write one myself. The case was easy to understand, as I was able to extract pertinent details to formulate the subjective, objective, assessment, and plan sections of the SOAP note for the patient. It was also interesting to see other SOAP notes from my classmates as everyone listed different information as to what goes into the objective part of the SOAP note. There are various findings from conducting a physical examination and other laboratory tests that we can put in the objective section, thus we all had variations in that aspect. Overall, I think that this case was great in practicing how to write a SOAP note, as we are required to write them in our other classes as well. It was a good introduction and I am curious as to how my SOAP notes will evolve as we progress further into the didactic and clinical phases.
